TEAM Registration

If your team got what it takes to win the league, don't wait and register your team now!

In order to register your team for the next available session, please contact Miroslav Mrstina via phone (708-870-7080) or email (This email address is being protected from spambots. You need JavaScript enabled to view it.), or in-person at MAX. 

Once registered for one of the sessions, you will need to submit a Roster and Release Waiver and Indemnity Agreement + COVID-19 Waiver. Failure to do so may result in forfeiting all games, until the forms are submitted. You can submit them via email (This email address is being protected from spambots. You need JavaScript enabled to view it.) or in-person before the first game.

During an active season, Miroslav can be seen running around MAX managing soccer matches. If you need to get his attention, strike him with a soccer ball. Coming up to him and saying "hi" works too!
